How to Sign-Up

Step 1 - Online Payment

Select the Type of Membership from the drop-down box below.

Enter your email address (must be valid and current).

If you're signing up for a Monash Student Membership, enter your Monash Student Number and Expiry Date. You may be asked to produce your Monash Student Card in class at any time for verification.

Annual Renewals are available only to former Members who have signed up for Semester or Annual Memberships in the past. The Kickboxing Club must have a record of your previous Membership to qualify for Annual Renewal.

Click on "Pay Now" button to make online payment by credit card or PayPal.

Once you have completed the
online payment, you will receive a confirmation email / receipt from
PayPal.

Print the confirmation email /
receipt and bring it to your first class.

The preferred method of payment
is by credit card or PayPal online. If you cannot pay online,
pay cash in class.

Note: If paying cash in class, arrive 30 mins early for your
first class to register and make payment.
Step 2 - Complete the Membership
Form
Download the Membership Application Form (pdf) and complete each
of the 5 pages (remember to have it witnessed on pages 2 and 4).

IMPORTANT: If you have any medical conditions that may be affected by training in Kickboxing, you must submit a recent Medical Certificate from your doctor stating that you are fit and able to participate in Kickboxing before commencing Kickboxing training.

If you are under 18, you must have parent/guardian consent (i.e.
your parent/guardian must sign the parent/guardian sections on pages 2, 3, 4).

This Membership
Form must be completed whether you are joining up for a
Trial Membership, a Semester or Annual Membership, or Annual
Renewal.

If you’re not sure
about a section, leave it blank and ask about it when you
bring it in to class.

If you are paying cash in class, determine the Membership you are joining up for and bring the
completed Membership Form along with the appropriate
Membership Fees (please bring correct change where possible) to
your first class (hand in to Committee Member).

Step 3 - Uniform

See a Committee Member in class to order and make payment for your
uniform.

For more information about uniforms,

Note: You don’t need to be in uniform right away if you’re
doing the Trial Membership, but will need to get your uniform within
2 weeks of signing up for the Semester or Annual Membership.
